61, lost her battle with lung cancer on March 6, 2013, at Memorial City Health and Rehabilitation Center in Houston, Texas.
She was a beloved wife, mother, grandmother, sister, cousin, aunt, friend and colleague who will be greatly missed. Sally loved reading, puzzles, movies and spending time with her family.
Sally is preceded in death by her parents, Silas and Nan Chisam; brother, Leland Chisam; and husband, Willard "Bud" Haynes.
She is survived by her brother, Michael (Denise) West of Toledo, OH; son, Michael Hayes, of Garland; daughter, Roni (Mark) Farabee, of Katy; step-daughter, Shellie Haynes; granddaughter, Sally Robinson of Katy; and many wonderful cousins and nephew.
A Memorial Service will be held at 2pm, Monday, March 11, 2013 at Charles W. Smith & Sons Funeral Home Chapel, 2925 5th St., Sachse, TX 75048.

In lieu of flowers, memorial donations may be made to the American Cancer Society or the charity of your choice.
